Before Your First Visit

Please review the information and fill out the paperwork below prior to your first session. This will save time and allow us to jump right in once you arrive!

Confidentiality Policy

The law protects the relationship between a client and therapist, and information cannot be disclosed without written permission. There are a few exceptions when this law does not hold.

These exceptions include:

  • If you threaten to harm yourself, I will make every effort to gain your cooperation in maintaining your safety. If you do not cooperate, I will need to take further measures to ensure your safety.

  • If you threaten to harm someone else, I must notify the police and inform the victim.

  • If there is suspected child, dependant adult, or elder abuse, action will be taken in notifying the appropriate authorities.

In the case of working with minors, parents have legal access to all information pertaining to their child. It is important, however, that older children and teens feel like their privacy is respected. Therefore, I encourage the agreement that besides general updates, information is only shared with parents when deemed necessary and after first speaking to the child about how we will go about sharing the information.

Fees and Cancellations

We will discuss fees when you schedule your first appointment. Payment is due at the time of the session. If you need to cancel or reschedule, you must do so within at least 24 hours in order to avoid being charged the full session fee.
